Altaddr is the same on my 4.0 farm server as the 4.5 server, which is in it's own farm. Other than what the external IP is set to that is. Local Address is default, alternate is the external IP.
I did figure out why I was getting a different error on quick launch. If I try to connect using the external name, I get the protocol error. If I try to connect via IP is when I get the server not available.

I don't think it does if you are doing a Quick launch. I still think this is Alternate Address related. If you are connecting internally then you would connect via the internal interface and you would not use the Alternate IP address. Since it is working from the outside via RDP you know the NAT is working, that and you can telnet to the NAT'ed address using ports 1494 and 2598. To me, everything points to the Alternate IP address. Try comparing the setting to the other servers in your farm that are working.
